CLICK HERE to Upload Case WORD FILE	PREFERRED OPTION Click this link to provide us with a full documented case in the form of a Word document, with all the patient's information and documenting pictures in the document. You will not need to fill out any more information if you choose this option.
CLICK HERE to use Case Study FORM	ALTERNATE OPTION Choose this option if you have separate pictures of a case.  We will have you fill out an outlined case documentation form that asks for the patient history, etc., and then you will be directed to upload pictures of the case. NOTE: We will only accept cases that are documented with photos. Thank you!
