This photograph depicts
a patient with acral lentiginous melanoma. Which one of the following terms
is used to designate the hyperpigmentation of the proximal nail fold?

A Auspitz sign
B
Nikolsky’s sign
C Hutchinson’s sign
D
Papular urticaria
E
Lichenification
|
Acral lentiginous
melanoma is an unusual form of malignant melanoma that
occurs on acral sites (palms, soles, nailbeds). This
form of melanoma is the most common form in black
patients. These tumors often have a poor prognosis,
because by the time of diagnosis, they have often
invaded deeply into the dermis. Hutchinson's sign
(pigmentation of the proximal nail fold) is a
nonspecific finding in some cases of acral lentiginous
melanoma. |

All of the following
antibiotic groups are generally known to be bactericidal except:
A aminoglycoside
B vancomycin
C fluoroquinolone
D chloramphenicol
|
Chloramphenicol is a bacteriostatic inhibitor of protein
synthesis. Penicillins, cephalosporins, vancomycin,
aminoglycosides, quinolones, and polymyxins are
generally considered bactericidal (ie, they kill
bacteria). Erythromycin, tetracyclines, chloramphenicol,
clindamycin, lincomycin, clarithromycin, azithromycin,
and sulfonamides are generally bacteriostatic (ie, they
slow bacterial growth). |
3 All
of the following may be associated with bacteria endocarditis except:
A
Osler's nodes
B
Roth's spots
C
calcinosis cutis
D
petechiae
|
calcinosis cutis (it's calcification in the skin as part
of CREST syndrome). Diffuse soft tissue calcification (calcinosis
cutis) is a rare complication of systemic lupus
erythematosus, and resembles the calcification that is
more commonly noted in systemic sclerosis and
dermatomyositis. It tends to develop in patients
diagnosed with SLE at a young age. For an intriguing
case study on this topic see:

4 Howell-Jolly
bodies may be present in patients with all of the following except:
A aplastic anemia
B
megaloblastic anemia
C
hemolytic anemia
D
post splenectomy
|
Howell-Jolly bodies are spherical blue-black inclusions
of red blood cells seen on Wright-stained smears. They
are nuclear fragments of condensed DNA, 1 to 2 µm in
diameter, normally removed by the spleen. They are seen
in severe hemolytic anemias, in patients with
dysfunctional spleens or after splenectomy. |

All of the following
sensory information is carried via the posterior column except:
A
vibratory sense
B
conscious proprioception
C
temperature
D
light touch
|
The
lateral spinothalamic tract carries sensation of pain,
temperature, and crude
tactile sensation. |
